    One of the most commonly sought out procedures in Dallas and Plano is the Brazilian butt lift.  This procedure involves performing extensive liposuction elsewhere on the body and using the removed fat to augment and lift the patient’s buttocks.  It is a well-tolerated procedure that generally takes about 3-4 hours.  Areas that undergo liposuction traditionally involve the inner and outer thighs, abdomen, love handles, and lower back.

    One of most important thing a patient needs to know before the procedure is the expected recovery time.  Patients are asked to refrain from lying on their back, so as to avoid pressure on their buttock, for 2 months after the procedure.  Patients are also asked to avoid directly sitting on their buttock for 2 weeks after the procedure.  After 2 weeks, patients are asked to place a rolled towel or a pillow under the back portion of the thighs, specifically where the hamstrings lie.  With this, the pressure on the buttock is off-loaded, and this is thought to help fat heal quicker after the operation.

    It is common for patients to negotiate an appropriate level of activity after undergoing tummy tuck, breast augmentation, liposuction, or any other combination of surgeries that make up a Dallas mommy makeover.  The thought is, if they can convince me that they need less time to recover than other patients, somehow the surgery will be lessened in magnitude, or that they will be able to recover more quickly after the procedure.

    Childbirth takes a dramatic toll on the female body.  To reverse those changes with a mommy makeover, significant surgery is involved.  With a Dallas tummy tuck (abdominoplasty), in addition to removing skin, extensive sculpting of the waist and upper abdomen is performed with liposuction, and the muscles of the abdominal wall are significantly tightened.  Undue straining for a period of time after surgery can disrupt the sutures, and lead to a less then desirable postoperative result.  In addition, straining after surgery can cause problems with wound disruption and bleeding, not to mention prolonged swelling in the abdomen, breasts, and lower extremities.

    I usually recommend that my patients take 1-2 weeks off of work after surgery.  Some patients will take less time and others more, depending on the level of physical exertion required at their work place.  I also strongly advise my patients to refrain from any vigorous exercise for 6-8 weeks after surgery.  Although it is important to walk almost immediately after surgery to prevent blood clots and respiratory problems, too much activity can lead to infection, bleeding, and wound healing problems.

    It is not uncommon to have patients come in seeking liposuction for major weight loss.  It is important to clarify with these patients, and to the general public, that liposuction is not a great option for weight loss.  Liposuction gives much better and more predictable results for patients close to their ideal body weight who are looking to sculpt their body. 


    If a considerable amount of weight loss is required, other surgical options are better.  Specifically, bariatric procedures, including LAP-BAND, gastric bypass, gastric switch, etc.  These procedures allow for much greater and predictable amounts of weight loss, which then can be followed by body contouring surgery to help sculpt the body.

    Awake liposuction is now available for patients at the Legacy Plastic Surgery Institute, the home of the Dallas Plastic Surgery Institute in west Plano. Tickle liposuction involves utilizing a European system known as Vibroliposuction. This involves using a small cannula, which utilizes nutational movements to cause very little trauma and pain during the procedure, allowing for it to be performed while awake.

    While it is a misnomer to think that this is not an invasive surgical procedure, it can be performed with patients being awake quite comfortably. Patients can expect to have between 2.0-2.5 liters of fat removed from the abdomen utilizing this system. The most commonly treated areas for this include the abdomen, love handles, thighs, and arms.

    Zeltiq and Zerona are two recently approved nonsurgical treatments for fat removal. We are proud to offer Zeltiq at EpiCentre in both Dallas and Plano. Zeltiq is excellent for selective removal of fat from the abdominal wall, flanks, and back. It utilizes a process known as cryolipolysis, which freezes fat. Patients can expect to see a reduction of up to 30%-40% of their fat over the span of a few months.

    The upsides of Zeltiq are that it is a nonsurgical procedure and patients can leave without significant pain, bruising, or discomfort. For each site treated, the procedure takes 1 hour. The amount of fat that can be removed with Zeltiq, however, is far less than can be removed with liposuction. However, liposuction is an invasive surgical procedure, and patients need to consider this when choosing between Zeltiq and liposuction.

    After virtually all types of liposuction, patients are asked to wear compression garments.  The rationale for this is that it helps the swelling improve faster after surgery, thereby allowing realization of the postoperative result faster.  In addition, in many areas of the body, compressive garments actually help the skin’s natural elastic properties work better, and can actually promote better skin re-contouring.  This is particularly true in the chin and arms.

    Most patients are more than anxious to get out of their compressive garment, as they complain about getting hot and sweaty, particularly during the summer months.  Nonetheless, failure to wear the compressive garments can affect the final postoperative results adversely.

    Many patients complain about fat upper arms.  It makes clothing choices difficult, particularly when contemplating wearing anything sleeveless.  Many patients come in wishing to know which contouring procedure is best to create a youthful and slim arm.

    It is important to differentiate between extra skin and extra fat of the upper arms.  Many arms have a selective excess of fat, and these patients are excellent candidates for liposuction alone.  Through very small incisions in the underarm and elbow region, the arm can be effectively contoured, and, with utilization of postoperative compression garments, very pleasing results can be noticed just a few weeks after the procedure.

    Other patients, particularly those that have lost large amounts of weight, tend to have extra skin.  They need some type of additional procedure to remove the extra skin, known as a brachioplasty.  In some versions of this operation, incisions can be limited to just the underarm area.  In the most complete version of this procedure, an incision is made down the length of the arm, usually along the inner service.

    Dr. Jejurikar’s goal is to minimize scarring while providing patients with the best postoperative result.

    Many patients come to the office inquiring about mini tummy tucks.  The rationale behind this is quite clear.  They would like a smaller scar, and believe that there is very little difference between a mini tummy tuck and a standard tummy tuck.

    There are key differences between the operations.  In a mini tummy tuck, there is very little separation of the skin from the abdominal wall muscles, meaning that less skin can be removed.  In addition, there is usually no tightening of the abdominal wall muscles, particularly in the abdominal wall above the belly button.  Therefore, if a patient has upper abdominal bulging or fullness, a mini tummy tuck doesn’t correct it.  Finally, there is no reshaping of the belly button with a mini tummy tuck.

    An advantage of a mini tummy tuck, however, is that, given there is less lifting of the skin from the abdominal wall muscle, liposuction of the entire abdominal wall and flanks can be performed much more extensively and much more safely.  This is because there is less chance of wound healing complications.

    Ultimately, mini tummy tucks are best for women that have undergone pregnancy and have only small lower abdominal pooches and only minimal to moderate changes to their abdominal wall.  It also can be good for men who have lost large amounts of weight.  For women that have bulging of the upper and lower abdomen with significant excessive skin and stretch marks, a full tummy tuck is almost always a better option.

    Buttock augmentation with fat, or the Brazilian Butt Lift, is being performed with increased frequency. The concept is quite simple; patients undergo liposuction in various locations and the fat that is removed is then re-injected to create a fuller, shapelier buttock.

    The most common sites for liposuction include the love handles, inner and outer thighs, abdomen and lower back. The fat that is removed is processed prior to reinjection to remove natural oils and serum. Even with this, the body reabsorbs some of the fat that is injected in the first few weeks after surgery. To compensate for that, if possible, Dr. Jejurikar overinflates the buttocks initially after surgery. To try to minimize the amount of fat reabsorbed, patients are asked to avoid placing any pressure on the buttocks for 10-14 days postoperatively. They are also asked to minimize sitting as much as possible and to pad the buttock while sitting for six weeks thereafter.

    I’m struck by the number of patients who come in seeking liposuction, but really need abdominoplasty surgery (tummy tuck).  There are important differences between tummy tuck and liposuction, which help determine which procedure is right for you.

    During liposuction, a blunt cannula is used to suction fat through multiple tiny incisions. Although many types of liposuction are available, including standard tumescent liposuction, ultrasound liposuction (VASER), power-assisted liposuction, tickle liposuction (vibroliposuction), etc., no skin is removed with any type of liposuction.  The best liposuction candidates maintain a healthy lifestyle and have focal fat deposits not responsive to diet and exercise..  Liposuction is not effective as a means for major weight loss.  Liposuction is not effective in treating skin with stretch marks, which are signs of irreversible skin injury, or muscle looseness, all of which is common after pregnancy or massive weight loss. In fact, in a patient who really needs a tummy tuck, liposuction may worsen the patient’s appearance. However, in patients close to their ideal body weight, without significant stretch marks, liposuction can be highly effective in treating extra fat of the love handles, thighs, back, arms, neck, calves and ankles.

    Abdominoplasty (tummy tuck) is usually a far better option for patients who have experienced massive weight loss or multiple pregnancies. Most of these women have extra abdominal skin with stretch marks, and separation of the abdominal wall muscles (rectus diastasis).  The muscle looseness gives the impression of an upper abdominal bulge and a lower abdominal skin apron. These women are great candidates for a tummy tuck, particularly if they are close to their ideal body weight. With removal of the extra abdominal skin, tightening of the abdominal muscles, and reshaping of the belly button, achieving a youthful appearance to the abdomen is often possible!
