A Brazilian butt lift is really two operations. Liposuction narrows the waist, flanks and back, and the fat that is removed is purified and grafted into the buttocks and hips. The waist gets smaller, the hips get fuller, and the silhouette changes more than either step could manage alone. It is one of the most popular procedures in my practice, and the one where I spend the most time talking about safety.
Safety First: Why I Use Ultrasound Guidance
The one serious risk unique to a BBL is fat embolism, which happens when fat is injected into or beneath the gluteal muscle and enters a large vein. It is rare, it is avoidable, and it is the reason the procedure earned a bad reputation a decade ago. The entire danger lives in one question: where exactly is the cannula?
I answer that question with real-time ultrasound. A handheld Clarius probe shows me the skin, the fat layer, the fascia and the muscle on a screen as I inject, so every pass of the cannula is placed in the fat above the muscle, where it belongs, and never guessed at. Ultrasound guidance is now required by law in Florida and recommended by the national plastic surgery societies; I use it on every BBL, and I will show you the images at your consultation.
The other safety rules are just as fixed. Your surgery is performed in an accredited surgery center with a board-certified anesthesiologist, I perform one BBL at a time with no overlap, and I will not graft more fat than your tissue can safely hold.
Safety Over Size
The results that age well are the ones that fit the frame. Over-filled buttocks look disproportionate, lose more of the graft, and put patients at risk. My goal is a smaller waist and a fuller, rounder buttock and hip that look like they always belonged to you. Most of that change comes from the liposuction; the grafting is what finishes the shape. If you want a size that your anatomy cannot carry safely, I will tell you so, and we will talk about what is achievable instead.
How the Fat Is Taken and Placed
- Lipo 360. Liposuction of the abdomen, flanks, lower back and, when needed, the inner thighs, so the waist is sculpted from every angle. This is where the hourglass comes from.
- Purification. The fat is processed to remove oil and fluid so that only healthy fat cells are grafted.
- Ultrasound-guided grafting. Fat is placed through small incisions in the buttock creases in many thin layers above the muscle, watching the cannula on ultrasound the entire time. Layering improves how much of the graft survives and keeps the result smooth.
- Hips. When the shape calls for it, fat is grafted to the hips as well, which rounds the silhouette and avoids the squared-off look of a buttock filled in isolation.

Am I a Candidate?
You are likely a good candidate if you are in good health, do not smoke, are at a stable weight and have enough fat to donate. Patients with very little body fat can still often get a meaningful result through aggressive waist liposuction and modest grafting, but expectations need to match the available fat. If you are taking a GLP-1 medication or still losing weight, we will time surgery for when your weight has settled, because the fat we graft needs to be fat you will keep.
What to Expect
Surgery takes two to four hours under general anesthesia and you go home the same day. For the first two weeks you will avoid sitting directly on the buttocks except on a BBL pillow and sleep on your stomach or side. Most patients return to desk work in about two weeks and to exercise at six weeks. Expect to lose some of the grafted volume in the first three months as the fat settles in; what remains at three to six months is permanent. The lipo areas continue to tighten for several months.
I use a non-opioid pain protocol, so most patients manage comfortably without narcotics, and I offer hyperbaric oxygen therapy at Foundry Hyperbaric and Wellness, which supports graft survival and reduces swelling in the first weeks.

Is a BBL safe? Performed with ultrasound guidance, in an accredited facility, with conservative volumes, it is as safe as other body contouring operations. The risk that made headlines came from fat injected into the muscle, which ultrasound eliminates.
How much of the fat survives? Typically 60 to 80 percent with careful harvesting, purification and layered grafting. I account for that when planning volume.
I am thin. Can I still have a BBL? Often, yes. The waist liposuction alone changes the silhouette, and even a few hundred cc per side placed well makes a visible difference. If there truly is not enough fat, I will tell you, and we can discuss alternatives.
Can a BBL be combined with a tummy tuck? Yes, and it often is, as part of a mommy makeover. The tummy tuck handles loose skin while the liposuction and grafting reshape the waist and hips.
Will the result look natural? That is the goal. Proportion, not size, is what makes a result look natural, and the photos above are the best illustration.
