Loose, hanging skin on the thighs is one of the hardest problems to fix without surgery. Diet and exercise shrink the fat, but the skin does not follow, and after significant weight loss, pregnancy or simply time, the inner thighs can rub, chafe and sag no matter how fit you are. A thigh lift removes that excess skin and fat and tightens the thigh so it looks firmer, smoother and more proportionate in shorts, a swimsuit or nothing at all.
A Thigh Lift Matched to Your Pattern of Laxity
Thigh skin loosens in different places for different people, so I do not perform one standard thigh lift. The inner thigh is the most common problem area, but the outer thigh, the front of the thigh and the whole circumference can all be involved. At your consultation I examine where the skin is loose, how much there is, how good the skin quality is and whether there is fat to remove along with it. Then I plan incisions that are as short as possible for the job and placed where clothing hides them.
| Technique | Best for | Incision |
|---|---|---|
| Mini (horizontal) inner thigh lift | Mild to moderate laxity confined to the upper inner thigh | Hidden in the groin crease |
| Vertical (medial) thigh lift | Significant inner thigh laxity from groin to knee, usually after major weight loss | Along the inner thigh, plus the groin crease |
| Outer thigh lift | Sagging of the outer thigh and buttock | Along the bikini line, often as part of a lower body lift |
| Liposuction alone | Excess fat with good skin elasticity and no true sagging | A few small puncture sites; see Liposuction |
In most thigh lifts I combine liposuction with skin removal. Thinning the fat first lets me remove more skin safely and gives a smoother, more even contour. The incisions are closed in layers with absorbable sutures buried beneath the skin, and I anchor the inner thigh lift to deep tissue so the scar does not drift downward over time.
Thigh Lift Results
Thigh contouring is frequently part of a larger body transformation after weight loss. You can see examples of combined lower body lift and thigh contouring in my gallery, and I will show you thigh lift results from patients with a starting point like yours at your consultation.

On the Beauty and the Beasts podcast, Dr. Sal Pacella and I talk about what weight loss and GLP-1 medications do to the skin, when fat grafting helps, and how surgeons think about staging body contouring after a big weight change.
Am I a Candidate for a Thigh Lift?
You are likely a good candidate if you have loose or sagging skin on the inner or outer thighs, your weight has been stable for at least six months, you are in good general health and you do not smoke. Thigh lift is most rewarding after major weight loss, whether through diet, bariatric surgery or a GLP-1 medication, once your weight has settled. If your main concern is fat rather than loose skin, liposuction alone may be the better operation, and I will tell you so.
What to Expect
A thigh lift is performed under general anesthesia and takes two to three hours, longer when combined with other procedures. Most patients go home the same day; a one-night stay is sometimes arranged for larger or combined operations. You will wear a compression garment for several weeks. Expect to walk gently from the first day, return to desk work in one to two weeks, and resume full exercise at six weeks. Swelling of the thighs and lower legs is normal and takes a few months to fully resolve. Because the inner thigh scar sits in a moving, moist area, I will see you frequently in the first weeks to keep the incision healing well.
I use a non-opioid pain protocol, so most patients manage comfortably without narcotics, and I offer hyperbaric oxygen therapy at Foundry Hyperbaric and Wellness to speed healing in the first weeks.
Combining a Thigh Lift With Other Procedures
- Lower body lift to address the abdomen, hips, outer thighs and buttocks in one operation, with the inner thigh lift staged separately when needed.
- Arm lift, since arms and thighs usually loosen together after weight loss.
- Liposuction of the knees, hips or flanks to balance the new thigh contour.
- Tummy tuck when abdominal skin is the primary concern.
Thigh Lift FAQs
Where will the scars be? A mini thigh lift leaves a scar in the groin crease that is hidden by underwear. A vertical thigh lift adds a scar along the inner thigh, which is visible when the legs are apart but hidden when standing. Scars fade substantially over 12 to 18 months.
Can I just have liposuction? If your skin is elastic and the problem is fat, yes. If the skin is loose, liposuction alone will make it looser. The examination tells us which you need.
Will a thigh lift fix cellulite? It improves the appearance of the skin by tightening it, but it is not a cellulite treatment and I do not promise that.
How long do results last? The skin that is removed is gone permanently. Keeping a stable weight protects the result; a large weight gain or loss can stretch the skin again.
Can I have a thigh lift and a tummy tuck at the same time? Often, yes. For patients needing an extensive thigh lift and a body lift, I may recommend staging the operations a few months apart for safety and a better result.
